First Time Appointments and New Patient Forms
What can I expect at my first appointment?
When you arrive, one of our front staff members will welcome you and provide new patient forms to fill out-- you can also download them below and complete them before you arrive to your appointment. Once your paperwork is collected, we will bring you back for a consultation and discuss any questions or concerns you may have. We will take x-rays of your teeth and the Doctor will review them with you before doing a complete oral exam which includes TMJ and oral cancer screening. You will receive a routine cleaning and discuss a treatment plan to address any findings and further work you may need. At the end of your appointment, the front staff will schedule future appointments and go over your insurance and payment information.
What insurance do you accept?
We accept many dental insurance plans and our team will provide all the forms necessary to apply your dental benefits towards your visit and any dental work you may need. You can find accepted insurance plans on our insurance page. If you don't see your insurance provider listed or have any other insurance related questions, please call or email us and we will be glad to help.

Please be aware that your policy coverage is an agreement between you and your Insurance company and that you are responsible for your total obligation should your insurance benefits result in less coverage than anticipated.
